This sheet is printable and explains the Sick Leave Bank.
This must be completed by September 1 or February 1 of the first year you join the Sick Leave Bank. You will remain a member as long as you are employed at USD 266. If the SLB gets low on days, you will be asked to contribute 1 day to remain a member.
Do not complete this form every year.
Complete this form to request additional leave days if you have an acute catastrophic illness or injury (of member, spouse, or child) and have depleted all of your chargeable leave days. Please note: Requests for days must be made immediately following a leave day docked in pay and prior to the next payroll run. Once docked days have been run through payroll, they cannot be reimbursed by the Sick Leave Bank. Days may not be “banked” for future use.
